Tennessee presents pronounced barriers to publication for early-career researchers examining healthcare equity. The state's 95 counties contain 35 designated primary care health professional shortage areas, yet only 12 percent of recent peer-reviewed articles on TennCare enrollment disparities originate from in-state authors without prior NIH funding.
Residents of the Cumberland Plateau and West Tennessee Delta counties travel an average of 47 miles farther than Middle Tennessee residents to reach the nearest academic medical library. This distance compounds when researchers must obtain de-identified claims data from the Tennessee Department of Health, which still requires physical notarization of data-use agreements.
Funding addresses these barriers by covering notary and courier fees for data-use agreements, as well as licensing of the Tennessee Hospital Discharge Data System for the single calendar year of the award. Applicants must reside in or be employed by an institution within one of the 35 shortage counties or demonstrate that their sample draws at least 40 percent of observations from those counties.
Application materials require a letter from the Tennessee Department of Health confirming that the requested dataset extract will be released within the grant period. Reviewers reject proposals relying on national datasets such as HCUP that do not allow county-level stratification for Tennessee's rural zip codes.
Unlike Kentucky programs that permit multi-state claims analysis, Tennessee reviewers enforce a single-state geographic restriction to maintain direct relevance to state Medicaid policy adjustments scheduled for 2026.
