Grant Overview
Tennessee’s 2023 State Plan on Aging projects that 1.4 million residents will be 65 or older by 2030, with the fastest growth occurring in the 15 counties east of the Cumberland Plateau. These counties contain 38 percent of the state’s rural road miles yet only 11 percent of its public transit seats. The Tennessee Commission on Aging reports that 27 percent of seniors in these counties live alone and have not driven in the past year.
Home-modification contractors are concentrated in the Nashville and Knoxville metropolitan statistical areas, leaving an average 87-mile travel distance for residents of Clay, Pickett, and Fentress counties. Medicare Advantage plans operating in Tennessee do not reimburse non-medical transportation, creating a documented gap between hospital discharge and safe return to unmodified homes.
Applications must therefore include memoranda of understanding with at least one Area Agency on Aging and one regional transit authority that together cover the proposed service area. The state also requires evidence that modification crews will use vehicles rated for gravel roads, a condition not imposed in West Tennessee’s flatter terrain.
Reviewers further examine whether the proposal identifies specific census block groups where the percentage of cost-burdened senior households exceeds 35 percent. This metric ensures resources target counties whose property-tax base cannot support local levy-funded aging programs.
