Grant Overview
Tennessee maintains 95 county health departments whose budgets are set by separate legislative bodies. Low-income residents in Appalachian counties face primary care provider ratios of 1:4,200, compared with 1:1,900 in the four largest metropolitan counties. Risk and compliance requirements for community health outreach therefore begin with county-level data on uninsured rates and documented gaps in preventive screening.
Tennessee's Medicaid expansion status creates separate compliance tracks: outreach in expansion counties must coordinate with managed care organizations, while non-expansion counties route participants through limited charity-care networks. Providers must maintain separate reporting templates for each of the state's three perinatal regions because birth outcome data systems are not centralized.
Applicants must also satisfy the Tennessee Department of Health's requirement that mobile screening units carry current Certificates of Need for each county visited, a process that can extend six months. This county-by-county certification differs from the regional permitting used in Kentucky and North Carolina.
